Pet Shield Flex Inc. — Pet Health Coverage Agreement (Lifetime)
A. Schedule of Benefits — Lifetime
Annual coverage limit: Unlimited for the natural life of the covered pet. Reimbursement rate: 90% of eligible vet invoice, 100% on instant-payout claims. Deductible: $0 (waived) permanently — one payment covers the pet's lifetime. Waiting periods: 1 day for accidents; 7 days for illness; 30 days for dental and chronic care. Add-ons (travel protection, lost-pet reward fund) you selected at checkout are itemized on your receipt and are governed by the same terms and exclusions as the base plan.
1. Parties & Effective Date
This Pet Health Coverage Agreement (the “Agreement”) is entered into between Pet Shield Flex Inc., doing business as PetShield Flex (“Pet Shield Flex Inc.”, “PetShield”, “we”, “us”), and the person identified in the signature block below (the “Policyholder”, “you”). The Agreement becomes effective when (a) you electronically sign this document, and (b) your first premium payment is successfully captured by our payment processor. Coverage begins at 12:01 a.m. local time on the day after the effective date, subject to the waiting periods stated for the plan you selected.
2. Premium & Billing
You authorize PetShield, through its payment processor (Stripe), to charge the payment method you provide for the recurring premium of the plan you selected at the cadence (monthly or annual) you selected at checkout. Premiums are charged in advance. If a charge fails, we will retry up to 4 times over 14 days; if all retries fail, coverage will lapse on the last paid-through date and no claims arising after that date will be reimbursed.
3. Cancellation & Refunds
You may cancel at any time from your dashboard. Cancellations take effect at the end of the current billing period; we do not pro-rate partial periods. You are entitled to a full refund of the most recent premium if you cancel within 30 days of the effective date AND have not submitted a paid claim during that window (the “30-day money-back guarantee”).
4. Claims Process
Submit claims through the dashboard within 90 days of the date of service. You must upload an itemized invoice from a licensed veterinarian and a brief description of the incident. Claims are reviewed by our AI adjuster; claims under $500 that meet our automated approval criteria are paid within 24 hours. Larger claims are routed to a human reviewer with a decision target of 5 business days.
5. Exclusions
This Agreement does not cover: (a) any condition pre-existing as of the effective date, including conditions you disclosed in the intake quiz; (b) elective procedures (cosmetic, behavioral training, breeding-related care); (c) costs incurred outside the territory permitted by your plan; (d) care provided by anyone who is not a licensed veterinarian; (e) any loss caused by your willful act, fraud, or material misrepresentation in the intake quiz or a claim submission. Misrepresentation voids coverage retroactively and obligates you to repay any prior payouts.
6. Communications & Electronic Signature
You consent to receive all policy documents, billing notices, claim updates, and required disclosures electronically at the email address you provide. You agree that your typed name in the signature field below constitutes a legally binding electronic signature under the U.S. ESIGN Act and equivalent EU/UK regulations. You confirm you have downloaded or printed a copy of this Agreement for your records.
7. Privacy
We collect only the information needed to underwrite, bill, and adjudicate claims. We do not sell your data. Full details are in our Privacy Policy, incorporated by reference into this Agreement.
8. Dispute Resolution
Any dispute arising out of this Agreement will first be addressed in good-faith negotiation. If unresolved within 30 days, disputes will be submitted to binding arbitration administered by JAMS under its Streamlined Rules, seated in Delaware, USA. You waive any right to a jury trial or to participate in a class action. Nothing in this clause prevents either party from seeking injunctive relief in court for misuse of confidential information.
9. Governing Law
This Agreement is governed by the laws of the State of Delaware, USA, without regard to its conflict-of-laws principles, except where mandatory consumer-protection rules of your country of residence apply.
10. Entire Agreement
This document plus the Schedule of Benefits for your selected plan (Section A below) and the Privacy Policy constitute the entire agreement between the parties and supersede any prior representations.

What if the terms change later?+
You stay on the version you signed until renewal. At renewal you'll see a side-by-side diff of any changes before you re-sign.
